Event Planner :: At Your Request DJs

This page contains three types of event planners, a reunion, corporate fuction and a wedding planner. We have provided hard copy of each saved as a word document. These files can be downloaded for you to complete, fax, mail or email as an attachement.

Please fill this event planner out and submit it three to four weeks prior to your event. The songs that are listed are only a reference for you, if there is a different song that you would like please enter it in the other spot.
